DFA Passport Application Additional Requirements: Minors Below 18 Year of Age
- Personal appearance of minor with both parents or mother .
- Personal appearance of minor with mother (if minor is of illegitimate status).
- Personal appearance of mother (if minor is 7yrs old & below).
- Passport of person traveling with the minor, original and photocopy.
Please note also that these are just additional requirements aside from the standard DFA passport application requirements as listed in the DFA Philippines website.

I ask my daughter 15 yrs old to apply for PHIL> Passport but her situation, she is an illegitimate child but her grandmother stand as her guardian because i live in Taiwan. What documents she need to provide to support her passport application?

what if a single mother is in abroad?and the passport of her child is for pick up already?
sir/ma'am,meron po akong passport kya lng nsa agency po,ngayon wla po kc akong pmasahe papunta manila para kunin at wla kc akong pantubos sa pasport k,now pwede po b ako kumuha ng bago kong passport kahit valid pa ung passport ko til september 2013?
just want to ask... what are the requirement if the minor is only 1 year old and 8 years old?
may i ask what if the child is leaving with her aunt,we even don't know where her father and mother is...
