DFA Passport Additional Requirements for Minor Traveling without Parent

• Original and photocopy of DSWD Clearance
• Affidavit of Support and Consent
• Personal appearance of mother (if minor is 7yrs old & below).
• Passport of person traveling with the minor, original and photocopy
