DFA Passport Application Requirements: New Passport Applicants

Many of you out there have been asking prior to coming over and heading to the Department of Foreign Affairs (DFA) what the passport application requirements are. I'd like to put emphasis that as of now, there are actually 3 categories for a passport applicant and based on those 3 categories either which one you fall have its own corresponding DFA passport requirement.

This post is all about the NEW PASSPORT APPLICANT and the required papers, documents and must do's for you in the process. Please that the following list of requirements were actually taken from the official website of DFA to avoid confusion and misinformation.

REQUIREMENTS FOR FIRST TIME PASSPORT APPLICANTS

  1. Personal Appearance is required.
  2. Duly accomplished application form.
  3. Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO); for born 1950 & below, Negative Birth Record from NSO & Joint Birth Affidavit from Two Disinterested Persons if no record from NSO.
  4. Three (3) colored photos of the applicant taken against a royal blue background & no shadow. Applicant should be in decent attire with collar & sleeves and in dark-colored shirt. Photo must be studio taken & of good quality, and must have been taken within the last six (6) months. Photo size: 4.5 cm x 3.5 cm. Facial image size: About 3cm-tip of head to chin; with 8-10mm head clearance from top edge. Ears and forehead should be visible. Wearing of accessories, colored contact lenses or eyeglasses, with name/name board are not allowed. DFA has the right to reject photos that do not comply with specifications and international standards.
  5. Original ID i.e. Senior Citizen's ID, Voter's ID, digitized government-issued IDs like SSS, PRC, BIR, Driver's License, original school ID (for students only) AND original supporting documents indicating full name, date and place of birth and citizenship (Please refer to the list below)
  6. If late registered, submit 2 original supporting documents indicating full name, date and place of birth and citizenship prior to date of late registration.
  7. NBI Clearance (not required if presenting 2 digitized Identification Cards.)
  8. Certified True Copy (CTC) of BC issued by the Local Civil Registrar if BC secpa is unreadable
  9. MC secpa of parents if Last & Middle name are foreign sounding (Chua, Ong, etc)
There are additional passport requirements however for a different case in this category and if you like to know more about them, please visit our other categories or the category which this post belongs.

Such would be:

  • For married women, who want to use surname of their spouse
  • For married women who would like to revert back to their maiden name
  • For Minors (below 18 years old)
  • If minor is NOT traveling with either parent
  • If traveling with father only
  • If both parents are abroad:
  • If Illegitimate, traveling with father
  • For Muslim applicants - For those whose births were registered
  • For Muslim applicants - For those whose births were not registered
  • For converts

paano po ba mag-apply ng online passport application for the first timer at magset ng appointment? i tried several times sa website nyo na online passport application pero di ako makaentry, could u pls. help me the procedure.thanks.

Hi po. Ask ko lang po about my BC SECPA wala po kasi akong middle name na nakalagay initials lang. Tapos po nirequire po akong kumuha ng BC sa munisipyo namin with complete data po. Is that okei to use it in applying for passport? Tnx po.

hi po, ask ko lang po kung pwde q makakuha ng passport?kasu po ung spelling ng name q s NSO, FATEMAR at s BIRTH CERTIFICATE ko FATIMAR.,late q na nalaman n FATEMAR ung s NSO q dahil since kinder hanggang ngaun FATIMAR ung gamit q.

I HAVE A 2 YEAR OLD CHILD WHO WAS BORN IN TH U.S. BUT I FAILED TO GET HIM A REPORT OF BIRTH IN THE PHILIPPINE CONSULATE IN LOS ANGELES. WE WOULD LIKE TO APPLY FOR A PHILIPPINE PASSPORT FOR HIM AND THAT IS ONE OF THE REQUIREMENTS. COULD YOU PLEASE HELP US ON HOW TO GET REPORT OF BIRTH FOR HIM HERE IN MANILA SO THAT WE CAN APPLY FOR HIS PHIL.PASSPORT? THANKS SO MUCH!!

hello..ask ko lang po what if i don't have any digitized ID's. what i have right now is only my POSTAL ID and my Voter's Certification from COMELEC.Is it okay for me to apply my passport? First time ko lang po kukuha ng Philippine Passport. Need your response. Thank you!

I'm Beverly Kim a daughter of Mrs NELY BERNARDINO ..My mom applied passport yesterday but they rejected for the reason of NO BIRTH CERTIFICATE,My mom was born in 1948..So sabi niyo kapag ipinanganak before 1950 you can able to get a "CERTIFICATE OF NON-AVAILABILITY OF BIRTH FROM NSO.." So Bakit po humihingi parin kayo ng Birth Cert? Kung merun naman siyang Certificate of Non-availability of Birth from NSO..?

Can I ask if how could I set my appointment with your office on line? coz I tried already but it doesn't work..pls. help me, Is the NBI, Confirmation Certificate, BC from NSO, and F-137 High School were already enough proofs to get the paasport?

Hello po.. ask ko lng po kung paano kukuha ng copy ng Local birth certificate kung and BC ay destroyed na..kumuha po kc ako ng Local BC sa municipal nmin, pero ang sabi destroyed na..Ginawan nila ako ng certification stating my BC was destroyed..Ng ibigay ko sa DFA ayaw nila tanggapin, gusto pa reconstract ko un Local BC ko, which is destroyed na nga..Un na lng po kc ang kulang sa mga documents ko.. Pwedi po ba ung Baptismal certificate.. Pls. help nman po.. Kailangan ko po tlaga ng passport.. Salamat po
