DFA New Passport Application: For Married Women Who Want to Use Maiden Name

If you are a married woman for example, with a foreigner and would like to use your maiden name in your passport application, below are the following additional requirements that you should provide the DFA. This is according to the list that I just found out from the official website of DFA Philippines.
Requirements For Married Women who Want to Use their Maiden Name
  • If already widowed: Death Certificate of the deceased husband and Marriage Contract (MC) in Security Paper issued by NSO or by the Local Civil Registrar duly authenticated by NSO.
  • If marriage is annulled: Certified true copy and photocopy of the first page and the dispositive portion of the judgment on annulment and NSO-issued Marriage Certificate with the annotation on the annulment decree;
  • If divorced: Certified true copy and photocopy of the first page and the dispositive portion of the judgment on divorce authenticated by the Philippine Embassy or Consulate where the divorce is obtained and NSO-issued Marriage Certificate with the annotation on the divorced decree.

paano po kung ang gustong gamitin ko ay ang maiden name ko.???- d po ako widowed,annulled,and divorced..pwede po ba////??????
